You will also be asked to give your reasons for changing doctors.

To see if your application is successful,after 2-3 days please ring the surgery between 12-1pm or 5-6pm to speak to a senior receptionist.

Practice Area

A map of the practice area is available in reception.

The practice covers an area of approximately 3 square miles from Oldbury Road in Rowley Regis to Quinton Road West (A456) and from Long Lane in Hurst Green to Thimblemill Road in Smethwick.

Since moving to Brandhall we no longer accept patients from areas beyond these boundaries, although we still retain some patients outside the area who were registered with us prior to our moving premises.

Reception staff will assist if you have any queries regarding the new area we now cover.

Since April 1999 the practice has been included in the governing arrangements for the NHS and is now part of Sandwell Primary Care Trust. We are, therefore, a Sandwell responsible general practice even though we have patients living in Sandwell, Dudley and Birmingham residential boroughs.

In light of the current COVID-19 crisis and to reduce footfall in the surgery, you may download our New Patient Registration Form and email it into us or post it to us with a clear photograph or scan of your photo ID + proof of address if available. For children under 5 and newborns, we also require a copy of the first page of their Red Book.
